Help Needed ...

I've been asked by our website host, Gamerlaunch, to ask you guys if you would mind trialling a new questionnaire for them.

Currently it's in beta and they would like feedback about any of the questions, format, etc.

It is a very short questionnaire .. most questions answerable by simply selecting an option.
